How to setup Host for remote access

Yes, this is the address the outside world sees you as being at.

That depends on your ISP.  Generally they're automatically assigned and
could well change (especially if your modem is not left permanently
on).  To get a more permanent address you should look at registering
with one of the dynamic DNS providers (e.g. www.dyndns.com) - that'll
provide you with a static hostname you can use.

Yes, you only have one address issued to the external device (i.e. the
modem/router).  In order to access anything behind this you'll need to
set up port forwarding on it.
