Forwarding a VNC Session

I've got  RealVNC server running at work.
When at home, I need to VPN into my work site, then I can open a VNC session
on my laptop.

Is there a way to forward, or daisy-chain, that exact session over my home
network?  The problem is that my workstation is dual-head, laptop has low
resolution, and my home setup is dual head.  I would like to be able to work
from my home machine, just as if I'm in my office.

I cannot VPN into work from my home computer, so the VNC session would have
to somehow be forwarded.

Does anyone know if this is possible?
